What is File of Life?

The File of Life is an information card that contains an individual’s medical and surgical history, medications, allergies, physician as well as emergency contact information. One copy of the card is located in a magnetized pouch on the OUTSIDE of the participant’s refrigerator, the other wallet size File of Life is placed in his/her wallet. A File of Life sticker is affixed to the door of the participant’s home to alert emergency responders of involvement in the program.

The File of Life is FREE

What information do you need to provide?

1. All current prescription bottles
2. The name and phone numbers of your physician(s)
3. The name, addresses and phone numbers of your next of kin
4. Any documents that list your medical & surgical history

How does one obtain the File of Life?

Stop by the Ambulance Corps building at any time and we will gladly provide you with the card and answer any question you may have. If any time you see one of our crews in a non-emergency situation, feel free to ask them and they will be able to provide you with the card as well. Otherwise, stop by the Ambulance Corp's table at many town functions including the Bike Helmet Distribution, annual Blood Drive, Health Fair, and many other events.
